KSC Registration forms (not needed at Tryouts)
For registration you will need:
1. Registration form (below)
2. Player photo for ID card
3. Birth Certificate if new to KSC
4. Player Code of Conduct form
5. Medical Release form
6. Payment
7. Uniform size needs (Order from www.teamgazelle.com)
