Coach / Manager Application
Anyone interested in managing or coaching a team for baseball or softball must complete a manager/coach application and sign the coaching agreement. Both can be downloaded, completed, and mailed to our PO Box. You can also print them out and bring them to a live registration or mail them with your volunteer application.

Team
The medical release form must be printed out by each manager and filled out by the entire team.
The manager evaluation can be completed by any parent who would like to provide feedback on the manager and coaches from the past season. The form must be printed, filled out and sent to: P.O. Box 82661, Kenmore, WA 98028
