To attend a Kajumulo FC practice, please fill out the Registration Release form.

To attend a Soccer Monster camp, please fill out the Soccer Monster Release form.

Players need to fill out the annual Concussion Compliance form, and US Club Medical Release and Parent-Player Agreement when joining the club.
