Fill out online, then print and bring the forms with you to registration! This will save you time at registration if your forms are filled out ahead of time. If you are filling out a volunteer form, you MUST also include a signed and dated Code of Conduct form.
Medical release forms will be collected by your child's manager at the first practice.
