These forms can be downloaded and printed off, signed and turned in to your team manager (or mail to JYH at POB 4733, Jxn, 83001 and we will give to manager). Managers will also have forms available for parents to sign at the first sessions. ALL forms must be completed prior to the skater stepping on to the ice! Thank you for your help in getting the paperwork completed.
