Things required for District play
For the players age 9 thru 18
If you tryout for one of the teams:
You must have a copy of your Birth Certificate
and 3 proofs of Residency.
Residence shall be establised and supported by
one document from THREE or more of the following
categories to determine residency of such
parents or guardian.
*It cannot be 3 from the same category.*
1. Driver's License
2. Voter's Registration
3. School Records
4. Welfare/cild care records
5. Federal records
6. State records
7. Local (municipal) records
8. Support payment records
9. Homeower or tenant records
10. Utility bills (gas, electric, water/sewer,
phone, heating,waste disposal.
11. Financial(loan,credit,investment,)records
12. Insurance documents
13. Medical records
14. Military records
15. Internet, cable or satelite records
16. Vehicle records
17. Employment records

Any one that is on a regular season team in the league for Minors 9 and 10 years of age as of April 30 and Majors 11 and 12 year olds can not be older that 12 as of April 30, 2009 . That lives in Huber Heights can tryout. You must bring your birth certificate and 3 proofs of Residency (where you live). examples: (1 utility bill, 2 car insurance, 3 car registration, 4 drivers license, 5 cable bill, 6 voter's registration card, 7 Employment records, 8 Welfare/child care records, 9 State records, 10 Federal records, 11 school records ( report cards are only good if it shows the address.) You will need to have these items in order to tryout if you don't make the team the items will be returned to you when the last try out is held. If you make the team all items will be returned after they have been checked by the district on June 21. If you make the team there will be a fee to be determined later.
