2012 Coaches Application

This form must be filled out annually by any coach, team parent, board member, and umpire. A nationwide background check is done on every volunteer each year.
A copy of a VALID GOVERNMENT ISSUED PHOTO IDENTIFICATION MUST BE ATTACHED with form. Back ground checks are run thru Lexis Nexis your Social Security number must be on the form or you will not be considered.

When you sign your registration form this is the parent code of conduct that you are agreeing to follow while participating with Little League baseball.

Pitching log is to be used at each game in order to make sure pitchers are only throwing the allowed pitches for there age.

Pitching Eligibility form is to show when a pitcher pitched and when they may pitch again.
