1. Completed Registration Form

2. All players must have physicals signed by your family a physician.

Pop Warner will accept:

a. 2009 Registration Form signed by your physician.

b. A note from your physician stating you are eligible to play. or

c. The 2009 Pop Warner Physical completed by your physician

3. Completed Medical Authorization Form

4. A “Certified” Birth Certificate. Photocopies NOT acceptable. Also please keep in mind that the birth certificate will not be returned to you until the season is over and ALL equipment has been returned.

5. A photocopy of end of year Report Card. The photocopy will not be returned. Home
schooled players and players that do not have a 2.0 average must complete the
Scholastic Eligibility Form.

The High Point Lions are always in need of volunteers for Pop Warner Football and Cheerleading. If you'd like to coach either football or cheerleading, become a team mom/dad, or numerous others join us. To be a volunteer for the High Point Lions we first need to know what your area of interest is and then we need the following information turned into us:

• Completed Volunteer Application

• A legible copy of your valid Drivers License

• We will then conduct a Background Check
