2009 Registration
This page describes what information is needed to register a participant into Hillside Pop Warner Football League, Inc. They are to be printed out and completed for submission at registration. Pop Warner Little Scholars, Inc. is a national organization with stringent guidelines for registration. Pop Warner Little Scholars, Inc., along with the North Jersey Pop Warner Football League, requires very strict proof of each participant’s age and weight, as well as a Physical clearing the child for athletic competition in Football or Cheer. In addition, Pop Warner requires every player/cheerleader to maintain a satisfactory level of academic achievement in order to participate. Providing this information for each participant prior to the start of the season is mandatory. Participants will not be eligible to play/cheer or begin practice until the required documentation is received. There are absolutely no exceptions to this policy! At Registration, you are required to complete or present the following documentation, which can be downloaded via the Forms Link:
- 2009 Participant Contract (Signed by the participant and parent/legal guardian).
- 2009 Physical Fitness & Medical History Form - Pop Warner Little Scholars, Inc. requires each participant submit a competed Pop Warner Physical Fitness and Medical History form. The form must be completed and signed by a parent or guardian, in addition, the signature and stamp of a licensed state examiner (M.D., D.O., R.N., etc) is required and the physical must be dated after January 1, 2009. No other forms or notes will be accepted.
- 3 Copies of the Year End Report Card (2008-2009): Pop Warner Little Scholars, Inc. requires each participant to demonstrate satisfactory academic achievement as evidenced by their report card. Only Year End Report Cards are acceptable. Three Copies are required – One for the participant’s registration record, one to submit to the Eastern Region Pop Warner, and one for Pop Warner Little Scholars, Inc. (National).
(NOTE: Copies of Report Cards are not required for the Flag Program.)
- Original Birth Certificate and One Copy (New Players only) – We need to see the original birth certificate but will immediately return the document to you. We will keep the copy for our records. Returning players have their birth certificated on file.
- Wallet Size Photo of the Participant (2in x 2in) – Prior to each Football Game or Cheer Competition, each participant is certified. The photograph is a required part of each participant’s Registration Record. The photograph must not be more than 2 years old.
- Registration Fee - Due at Registration*** (Checks or Money Orders Only!)
- Flag Football - $70.00
- Early Registration - Cheer $110.00 and Football $135.00 (April 1 - June 30)
- Regular Registration - Cheer $150.00 and Football $175.00 (June 30 - Aug 30)

Participants will not be allowed to take part in any Pop Warner activities until the form has been submitted. There are no exceptions! Important Note: Participants registering prior to the close of the school year will have until Saturday – July 11, 2009 to submit year end report cards. The participant will not be placed on a team until the report card is received.
***If there is a prior outstanding balance for ANY reason (bounced check fees, 2008 Registration Fees, Unreturned or lost equipment etc., YOU WILL NOT BE ALLOWED TO REGISTER YOUR CHILD UNTIL ALL PAST DUE BALANCES HAVE BEEN SATISFIED.
