2009–2010 Travel Hockey Registraton Requirements
The following documents/forms will need to be provided and/or completed:
- Copy of birth certificate (for all new players).
- Current Cleveland Heights Recreation Department ID card (residents only)
- *Cleveland Suburban Hockey League release (if your child played in another program previous season); available from the director of your previous program.
Please note: Those wishing to try out for the CHHL will must pay a non-refundable deposit of $100.00 and complete all required paperwork before the first tryout session in August 2009. Deposits will be applied to the total fees once the tryouts are completed and teams are selected.
Age Classifications
Players are placed in age classifications determined by their age as of December 31, 2009. The age classifications for the 2009–2010 season are:
Mites: ages 8 and under
Squirts: ages 9 and 10
Pee Wees: ages 11 and 12
Bantams: ages 13 and 14

2009-2010 Travel Hockey Registraton Fees
Registrants will need two checks, one to cover the fees due to the City of Cleveland Heights and one to cover the fees due to the CHHL. Amounts due are listed below.
Fees Payable to "The City of Cleveland Heights"
Mites, Squirts, Bantam:
Hockey Fee: $220.00 (resident); $400.00 (nonresident)
ID Card: $5.00 (resident)*; $0.00 (nonresident)
Total City of Cleveland Heights Fee: $225.00 (resident); $400.00 (nonresident)
*Parent/guardian MUST purchase I.D. card.
Includes Developmental Hockey Camp Registration. Camp fees will be credited to hockey registration fees.
Multiple-Child Discount: $20.00 reduction for second child; $30.00 reduction for each additional child thereafter. Reductions apply to City of Cleveland Heights fees only, not to CHHL fees.

Mites: $135.00
Squirts: $150.00
Pee Wees: $165.00
Bantams: $180.00
Includes Cleveland Suburban Hockey League fees (referees, administration, etc.) and Lake Erie Mega Monsters Program.
Player Jerseys
League and city fees do not include home and away jerseys. If needed, purchase them from Logos On Lee, 3101 Mayfield Road, Rockefeller Building, Cleveland Heights, OH 44118. (216) 371-5734. Please check with your team manager for available uniform numbers before purchasing jerseys.
