General By-Law, Rules Of Operation & Board Positions

Revised and accepted by the membership at the AGM on April 10th, 2001. There are two document; the General By-Law which outlines the organizational structure of our association. Changes to this document can only be made at the member annual general meeting. One small change was made at the AGM in 2006 to recognize GVDMHA association with the OWHA. The Rules of Operation were published to help clarify how hockey operations are to be handled for all participants. Revisions to this document can be made by the board at any time during the season. They were also first introduced at the AGM in April 2001, and revised where necessary by the board in May 2002, April 2003, March 2006, February & September 2007.

GVDMHA Member Agreement

Registration 2003 brought the development of this member agreement. All families registering with minor hockey are expected to agree to the terms of this agreement and maintain the standards of behaviour laid out in the OMHA Code of Conduct.

Parental / General Concern & Compliment Form

The Grand Valley & District Minor Hockey Association is run entirely by volunteers, who donate their time unselfishly. We support the coaches and staff that have been installed. We know and you know that the time that coaches, assistant coaches, trainers, managers and board volunteers give of themselves is sometimes overlooked and underestimated. Many of us choose not to volunteer for various reasons. The success of our minor hockey program depends entirely on our volunteers and their level of enthusiasm. During the course of the minor hockey season, situations may arise that concern you. Remember to look at your situation with an open mind. Remember that the volunteers of our hockey system are looking after the interests of all the players, and teams, not just one player or team.

However, we do take your concerns seriously. If you have a concern, please download this form, print and sign your name in the appropriate spaces. Please write a short paragraph explaining your situation or concern and give it to any of our board members. It will be put on the agenda of the next board meeting. It will be kept in confidence and you may be asked to explain your situation in greater detail to the board at a future meeting.

On the flipside, please feel free to offer thanks and compliments via this form as well. If you like something we are doing, or think that one of our volunteers is doing a great job and should be recognized for such, please let us know.

Concussion Card

Any time a player participates in contact sport, there is a risk of injury. Concussions are one injury that can be found in hockey, but the symtoms are often missed or misdiagnosed. It is important for everyone involved: players, team official, parents and hockey administrators to be aware of the symtoms and effects of concussions. This handout is taken from the Hockey Canada website and is all about concussion awareness.

2011 - 12 Coaches Application

Applicants,

Thank you for your interest in volunteering for a position with the GV&DMHA.

We are now accepting applications for coach, assistant coach and trainer positions for the 2011/2012 hockey season. Although we cannot confirm at this time which teams we will field, we are accepting applications for rep teams from Novice to Midget, local league teams from Tyke to Midget, and our IP Program.

The attached application form has been developed in a manner consistent with the recommendations from the OMHA.   An interview may be required in addition to this application, before a position is offered. A volunteer application also requires that you submit a consent form for a criminal reference check. The association policy on volunteers and members with Criminal Records is attached.

Please note that this is just an application. It is the applicants’ responsibility to become familiar with the policies of GVDMHA. Section 6 of our Rules of Operation details the responsibilities and expectations of team officials. You can download a copy of the Association’s policies from our handouts section, or contact minor hockey to receive a copy.

If you are interested in coaching or helping with a team, please submit your application by June 28, 2011. Ensure that you have filled in the application completely and feel free to attach any additional information you feel is pertinent.

Coaches can select their remaining bench staff and submit to the executive for approval. Preference will be given to volunteers that complete an application. All members of the bench staff must be fully certified as required by OMHA as well as Prevention Services and Police Record Check before they will be approved on a roster or allowed on a bench during a game. Anyone with expiring certification in 08 or prior must update their certification before they will be approved on a roster.
