All coaches and volunteers participating in the Soccer Club of Guilford programs must submit a completed Connecticut Junior Soccer Association (CJSA) Volunteer Disclosure Form. Download the attached handout and mail it to the Soccer Club of Guilford, P.O. Box 21, Guilford, CT 06437.
