REQUIRED REGISTRATION INFORMATION:

* Players Birth Certificate

* 3 seperate documents that show the address of the legal residence of the parent(s) or court appointed guardian(s) for the player(s).   EXAMPLES: driver's license, utility bill, financial records (credit card bill, bank statement, insurance card).
