Dress Code

The attached form is the league policy regarding Parental Grievance Procedures. In the event a Grants Pass / Rogue River Pop Warner parent is unable to resolve an issue with a coach, the following league grievance procedure is to be followed. A reasonable effort must be made by the parent to communicate his/her concern to the head coach, before the grievance procedure can be initiated.

Jim Thompson, President, Grants Pass Pop Warner will investigate all grievances.

League Grievance Procedure

This form must be completed by a parent and the participants physician. No participant can begin Pop Warner practices until this form is completed. Physicals MUST be completed every year, per Pop Warner regulations.

2013 Physical Form

This application must be completed in full and have a government photo ID attached (i.e. drivers license). Note: Any person with a felony conviction can not hold any position in Oregon Pop Warner. Volunteer applications that reveal a felony conviction will be automatically denied.
