This is a Bethel School District letter that describes some of the risks associated with wrestling. It must be filled out and signed prior to first practice

Skin Form

This form is required to be filled out in it's entirety and signed by a physician if a wrestler is sent to a doctor for a suspected skin contagion. Make sure everything is marked correctly on the human figure, whether or not it's contagious, and a date is filled in when it will no longer be contagious.
