Your registration session will run smoothly and quickly if you download, print and fill out the following forms. 2011 Participants Forms
- Please download the adobe files & print on plain white paper. As the parent/guardian, please complete the "Xtreme Contract", "Participant Contract" & "Parents Code of Conduct". The "Physical Form" is to be completed by your family doctor with their "wet" signature & "wet" stamp. Once completed, please deliver all originals and three copies to our registrations table.
