2009 Player/Cheerleader Registration Forms
Please download the adobe files & print on plain white paper. As the parent/guardian, please complete the "Season Contract" & "Parents Code of Conduct". The "Physical Form" is to be completed by your family doctor with their "wet" signature & "wet" stamp. Once completed, please deliver all originals to our registrations table.

2009 Adult Personnel Registration Forms
The following forms are for Adult Volunteers 18 years of age or older. Please download the adobe files & print on plain white paper. Please complete the "300 Form" & "Personnel Code of Conduct". Once completed, please deliver all originals to our registrations table. Also, all Adult Volunteers must LiveScan (finger print) and be cleared by the Conference.
