Glen Este Trojans Athletic Boosters: Welcome
The Glen Este Athletic Boosters exists to generate support for the high school athletic programs. We work with the community to provide all students an opportunity to enjoy the high school athletic experience. Every family with a student participating on a Glen Este Athletic Team is automatically a member of the Boosters. This organization promotes the educational, physical and moral welfare of the GE students through direct involvement, financial, and moral support. To be successful, we need your help! We urge you to become involved in the Glen Este Boosters by supporting and promoting the special events that help all Glen Este students.
Please consider this your invite to our next meeting.

February 16, 2012 Dear Athletic Director, Parent, or Guardian, Once again it is that time of year to be planning pre-participation physicals for the 2012-2013, school year. The cost for the physical exam will be $10.00, and all athletes will also be given a
t-shirt. There will be no pre- registration or appointments. Physicals will be first come, first serve, with registration and payment at the door. The following table indicates the dates and times that Wellington Orthopaedic and Sports Medicine will be offering these exams.
| Date | Location | Registration Time |
| June 6, 2012 | Colerain High School | 6:00 – 7:00 pm |
| June 9, 2012 | Wellington – Anderson Location 7575 Five Mile Road Cincinnati, OH 45230 | 8:00 – 9:00 am |
The physical form requires a signature from a parent or legal guardian. Wellington also has a consent form that must be signed. We ask that the parent or legal guardian be present for the physical exam. If they cannot be present, then the athlete must present the signed consent form upon registration for us to perform the exam. If a parent or legal guardian is not present, and the athlete is under the age of 18 and does not have a signed consent form, then we will not be able to perform the physical exam. We encourage everyone to use their family physician or pediatrician for this examination. This is merely a service provided to the community by Wellington Orthopaedics. Also remember, that Federal Law now requires a signed letter by the parent or guardian to release the physical form to the school. This is a known as a HIPPA consent form, which also requires a signature from the parent of the legal guardian. This must be attached and on file with the physical form. Please make sure those attending have the appropriate, signed documents or a parent available to sign these on the night of the physical exam. The school can only collect the forms with the appropriate signed documentation. The parental consent forms can also be found electronically on our website at: http://wellingtonortho.com/therapy-services/athletic-trainer-services/upcoming-events An updated copy of the physical form and HIPPAA form can be found at your school’s athletic department or on the following OHSAA website. Please note that this form is school year specific: http://www.ohsaa.org/medicine/PPE.pdf Please feel free to call me if you have any questions or concerns. I can be reached at 513-245-5434. Thank You,Lisa M. Sweeterman
