The white copy of the 3 part game sheet will be taken by the referees.  It is the responsibility of the offending team to provide the referee with a self-addressed stamped envelope.  The envelope should have the following address on it:

In addition to this, the offending team manager/coach,  must fax or email a copy of the game sheet that they received after the game ( either the yellow or pink copy) to the OWHA Director of Discipline, Doug Nicholls and a copy should be sent to the OWHA Regional Rep., Debbie MacDonald, within 48 hours.  Failure to do so could result in further suspensions.     In order to assist a coach in understanding the penalties, it is  recommended a copy be sent to the MOGHL Director of Discipline.  

Please note, that OWHA must be notified, by faxing a copy of the each game sheet showing ( suspension 1 of xx ) beside the players name.  Usually the referee will initial this.  An email to the MOGHL Director of Discipline should be sent, indicating the game dates that the offending player/coach sat out.
