PLAYER EVALUATION FORM
All coaches are required to complete the Player Evaluation Form before their team is allowed to participate in the end-of-the-season tourney.

The ratings of each player should be based on the best overall player(s) in a particular division. Please do not rate your players based solely on a comparison amongst players on your individual team as this will not give a true indication of their overall rating.
