Hockey Initiation Program(HIP)
Every WEDNESDAY at THE CUBE
2009-2010 season beginning October 14, 2009
6:30PM - 7:20PM
This is for beginning skaters and those wanting to learn to play HOCKEY
born in 1997 or later (aged 12 and under)
PROGRAM IS FREE!
NO SKATING EXPERIENCE NECESSARY!
Full equipment required - Hockey equipment available for a $100.00 deposit.
(sticks & skates not included, but skates can be rented from the Cube)

Please proceed to the "Registration" section to register your child.
Registration with USA Hockey is necessary and FREE for HIP.
Copy of birth certificate must be provided with registration.
FAHA Registration Guidelines:
If you would like to play youth ice hockey with the Findlay Amateur Hockey Association, please complete the registration paperwork, available by clicking the links below.
NOTE: ALL FIELDS MUST BE COMPLETED AND ALL FORMS SIGNED BY BOTH THE PLAYER AND THE PARENT IN ORDER FOR REGISTRATION TO BE COMPLETED. In the event your child is too young to sign his/her own name, you may sign it for them.
1. Register online with USA Hockey. Go to www.usahockeyregistration.com to do this.
2. Choose member type "Ice Player/Coach"
3. Check the box that acknowledges that YOU are 18 years old.
4. Click "continue" on the 2009-2010 season
5. Follow the prompts...fill in the data correctly.
6. Process payment - Visa, Mastercard, Discover or American Express ( credit or debit card) A Visa gift card will also work.
7. After successfully completing online registration you will receive a confirmation page. You can email your confirmation page to Chris Woodward, FAHA Registrar, at dcsjbw91@aol.com at this time.
8. From the FAHA website, open, print and complete the Consent to Treat Form (now and editable pdf.) and print.
9. From the FAHA website, open, print and complete the Code of Conduct Form.
10. From the FAHA website open, print and complete the FAHA registration form.
11. If your child was not born in the United States then open, print and complete the Youth Transfer form. There is a youth transfer form for youth's born in Canada and a seperate youth transfer form - international for youth's born anywhere other than the USA or Canada.
12. If you are new to FAHA, please send in a copy of your childs birth certificate.
13. Turn in forms to your manager, coach, USAH Registrar, Chris Woodward, or mail to FAHA, 2447 Tiffin Ave. Suite 213, Findlay, OH 45840.
