Volunteer Forms
FHLL REQUIRES every Board Member, Coach, Manager, Umpire (and any other registered FHLL volunteer who has frequent and direct interaction with the children on behalf of our Little League program) to complete the current Volunteer Application. FHLL also requires each volunteer to complete RCMP forms (Form 3584 ?Consent for Disclosure of Criminal Record Information? and take these forms along with photo ID (drivers license or passport) to the local branch office located at:

Please note there is no charge for this service by the RCMP
All forms must be processed and received by the Safety Officer no later than APRIL 30th.
