Fallbrook Pop Warner: Registration Information

Paperwork Turn-In Dates:  All required/signed paperwork must be submitted at an upcoming 'turn-in' date.   

**Next one is Wednesday, May 29th at the Fallbrook Community Center from 6:45-8:30pm** 

Registration Fees:  

Flag Football = $175
Flag Cheer = $175 (uniform included)
Tackle Football = $299 before June 1st - $349 after

Tackle Cheer = $325 (uniform included)

Payment plans are available.  

Required Player Documentation:

• Birth Certificate

  • All Players/cheerleaders are required to turn in a copy of your birth certificate with State Seal clearly legible.  Original Birth Certificate must accompany copy for verification purposes - original will be returned.

• 2012 Player Physical Fitness & Medical History Form

  • All Players/Cheerleaders are required to submit a 2013 Pop Warner Physical Fitness Form completed by a physician. The player/cheerleader will be not be allowed to practice if they do not have a completed Physical.  See the 'Forms and Handouts' section.  No cost physicals will be given to football/cheer players in July.  See the 'player information' tab for more information.

• 2013 Player Contract

  • All Players/cheerleaders are required to turn in a completed/signed 2013 Pop Warner Player Contract. The player/cheerleader will NOT be assigned to a team until the form is signed and received.  See the 'Forms and Handouts' section.

• Report Card 2013-2014

  • All players/cheerleaders must turn in 2 copies of their FINAL YEAR-END 2012-2013 report card. The player will be removed from their team roster if the report card is not turned in by July 31, 2013. The final year-end report card can be turned-in at Skill Evaluations, Physicals, Equipment Issue or mailed/emailed. 

• Website/Photo Release Form

  • All players/cheerleaders must sign the Fallbrook Pop Warner Website/Photo Release Form allowing FPW permission to post pictures of you on our website or in a local newspaper.

These forms WILL BE attached to your online registration receipt.
