All players and cheerleaders must have a physical exam by a qualified physician before participating in American Youth Football and Cheerleading League Program.
The physical exam form (downloadable in the Handouts section of this website) or a written statement from your own physician stating that the participant is physically able to practice and play football or participate in cheerleading must be completely filled out with one year prior to the first day of practice.
PLEASE NOTE: This form MUST be completed and returned to the organization by the first practice. Participation in the program is not permitted until this form is submitted. This rule is strictly enforced! If the form is not submitted, your child will sit on the sidelines until it is received. Please allow ample time for your physician's office to complete the form.
Parent Volunteer Requirement
Parent involvement is vital for the Packer organization to meet its overall needs. Parents are required to assist at the concession stand, handle chains and yard markers during games, monitor player activity, and supply fruit/snack for half-time or end of games. Each of these duties will be requested usually 2 to 3 times during the season (dependent upon roster size). You will be notified by the Director of Volunteers of the date(s) your family is assign to these positions. Please note that once your volunteer date and time is assigned it is an expectation that your family volunteer designee report promptly to his/her assignment. If your family volunteer does not show up your child may be asked to sit out of the game or cheer activities for that day.
Other areas needing help through the year include special activities such as community service projects, Parents' Night, and the banquet. Please see the person identified as the coordinator if you can help.
Cheerleading Uniform Deposit
A $50 deposit is required at uniform pickup and refundable at the end of the season.
