Cost is $ 35.00 for players born in 2002 and earlier.  Free to players born after 2003.

Step #2:  Download and Fill out Euclid Hockey Registration Form

Step #3:  Download & Fill out the Following Forms

USA Hockey Waiver of Liability Form

USA Hockey Consent to Treat Form

USA Hockey Participation of Conduct Form

EYHA Parent’s Code of Conduct Form

Step #4: 

                  Option 1:  Complete and mail forms along with your check made payable to ‘Euclid Youth Hockey Association’ to
