El Molino Little League: Welcome

We will not be able to accept an incomplete registration. If you do not have all of the forms and documentation required, we will not be able to accept the registration and will ask you to come back later with a complete registration.

What You Need to Bring to Registration: (All forms are attached)

1.     2012 Registration Form (2 pages)

2.      Copy of Birth Certificate (If you do not have a birth certificate please refer to the attached Proof of Age Requirement)

3.      Three Proofs of Residency: (Please bring copies that we can keep.)

Residence shall be established and supported by documents, dated or in force between February 1, 2011 and February 1, 2012, from THREE OR MORE of the following categories to determine residency of such parent(s) or guardian. For example three items from No. 10 below constitute only ONE document.

1. Driver’s License 2. Voter’s Registration 3. School records 4. Welfare/child care records 5. Federal records 6. State records 7. Local (municipal) records 8. Support payment records 9. Homeowner or tenant records 10. Utility bills (i.e., gas, electric, water/sewer, phone, mobile phone, heating, waste disposal) 11. Financial (loan, credit, investments, etc.) records 12. Insurance documents 13. Medical records 14. Military records 15. Internet, cable or satellite records 16. Vehicle records 17. Employment records

4.     Volunteer Application:

Since anyone who will volunteer in our league needs to have a background check and most of you will hopefully volunteer we would like you to bring the Volunteer Application filled out to registration.

5.     PCA Parent Pledge: 

Please read, sign and bring with you to registration.
