If your child is between the ages of 9-12
and is interested in Allstar's please open and fill out the following attachment and return it to the concession stand where there will be a box for the forms. The bottom of the form needs to be stamped and initialled by a board member or an adult working the concession stand. If it is not stamped/initialled, THE PLAYER WILL NOT BE ON THE VOTING BALLOT.
