  • Parent / Guadarian Driver's License
  • 3 proofs of residency of the parent(s) or court appointed guardian(s) 
  • Players New to the League must bring their Birth Certificates
  • Filled Out Registration Form
  • If paying with a Credit Card a 2.2% + .30 processing fee must be paid in cash or check at the time of registration
