Elk Grove In-Line Hockey: Referees Information

Below is the web site for the referee test that you must pass before you will be allowed to referee any games

Your results have to be in Charlie Pritscher's hands no later than Sept 9th

He will accept test results by e-mail or by dropping them off at his house.

You will also need to print out the referee independent contractor agreement form, along with the W9 form, fill these out and get them to Charlie.

If you are under sixteen you will need to fill out the work permit form bring it into your school have it filled out return it to Charlie.

All of these forms are required by the park district payroll department if you want to get paid for the games you ref.
