Registration Deadlines

Fall 2009 Registration is open NOW!

Early rate: $125 EC residents/$140 non-residents if postmarked by May 31 (U6-U14 ONLY)

Regular rate:  $140 EC residents / $150 non-residents if postmarked by June 15  

Late rate: $150 EC residents / $165 non-residents if postmarked after June 15  

Registration Instructions

Please complete and send in the following items:

1. TWO (2) copies of the CYSA Registration Form completely filled out IN INK and signed on each form. NO PHOTOCOPIES WILL BE ACCEPTED. ORIGINAL SIGNATURES ARE REQUIRED.

2. ONE (1) completed Liability Waiver/Emergency Authorization Form

3. ONE (1) completed Information Sheet

4. ONE (1) recent photograph approximately 1" x 1" with player's name on the back

5. ONE (1) copy of birth certificate or passport for ALL NEW EC F.C. players

6. ONE (1) check for registration fees, payable to EC F C.

7. ONE (1) refundable Parent Participation check for $75.00 (payable to EC F.C.)

The $75.00 fee is returned at the end of the season when your volunteer hours are completed.
