Registration is open for our 2nd annual Xcelerate Lacrosse Camp @ Evergreen State College (Olympia), August 3-7, 2008, for boys ages 10-17… www.xceleratelacrosse.com

We also have an Adventure Lacrosse Camp in Colorado, August 4-8, 2008!

WA State Patrol Clearance Form
Coaches, its time to obtain clearances from the WSP for the 2008 season. Please forward this mail to anyone who will be acting as an assistant coach and to team managers or parents who are assisting on the fields or sidelines, however frequent or infrequent, as they become known to you.

Please follow the link below, complete the form, and return it to me or send it to our box at mailpost # 544.

To complete the form:
A. The requesting agency is Lake Washington Lacrosse Association, dba Eastlake Lacrosse. You do not need to complete the rest of the box
B. Check the Non-Profit Business/Organization box.
C.   We need your full last, first and middle name, any alias, and your date of birth. We do not need race, SSN or Driver's license.
D. Please complete this section entirely.

If you have a common name, or if I informed you last year that there were names identical or similar to yours to clear, please list your height, weight, eye colour and place of birth at the bottom of the form.
