This is an individual copy of the medical release form that accompanies the player registration form. Every player must have a signed copy on file with Eastlake Lacrosse in order to play.

2009 Registration Form

Registration Instructions:
1. Complete this fillable form.
2. Print, then parent(s) and applicant/athlete sign, initial, and date. You must sign and return:
Registration form (pages 1 and 2)
Medical Release form (page 3)

Send payment in full, except for grades 9-12
Mail to: Eastlake Lacrosse 704 228th NE #544 Sammamish, WA 98074 Registration is not complete until the Registration Form, and Emergency Release Form are received.
