Recently, the state of Washington passed HB 1824, “The Lystedt Law,” which is a concussion and head injury management law. Within this law is a component for High Schools, which includes its coaches, parents, student-athletes and the athletic organizations which utilize the high schools.

This law states that “all athletes and the parent(s)/guardian(s) of those athletes sign and return an information sheet relating to the nature and risk of concussion or head injury.”
ALL PLAYERS NEED TO SIGN THIS FORM & RETURN IT TO YOUR COACH PRIOR TO THE FIRST GAME

Concussion Form

The Student Driver From must be filled out by each player who plans on driving themselves to/from games or driving other players in their car to/from games. A copy of proof of insurance is also required.

THE VEHICLE OWNER FORM must be completed in addtion to the Student Driver Form.

PARENTS who intend on driving players to/from any game or activity can complete the Vehicle Owner Form

Each Player on the Varsity/JV teams will be required to fill out and return this form so that they can obtain rides from other players or parent drivers to and from games within LWSD boudaries.

In the event there are games outside of the LWSD boundaries, Parent Drivers will be assigned to drive. Those parents need to fill out the Vehicle Owner Forms and turn them in prior to driving.
