2012RegistrationForm

This form is required for each family that opts to participate in the Payment Plan arrangement. This allows families to pay 50% of the amount due along with registration, and agree to pay the remaining balance prior to July 15th, 2012. You may download this form, complete it and mail along with your registration to P.O. Box 340084; Beavercreek, OH 45434.

2012PaymentPlan

    This is required for each individual wishing to volunteer in a head coach, assistant coach or team parent role.
    This can be submitted electronically to ebfcfootball@yahoo.com, in person at the registration rally, or mailed in to our PO Box above.
    Please submit your form by no later than April 29th!

2012VolunteerForm

    This is required for each football player and cheerleader.
    Please complete and submit with your registration, at equipment hand-outs or at paperwork check-in on the first day of practice.

2012EmergMedROL

    This is required for each registered football player and cheerleader.
    Each parent must sign and date the form. Those living in separate homes may submit one form per person, for the same child.
    This can be submitted at the registration rally, mailed to the PO Box above or in person at equipment hand out or at paperwork check-in on the first day of practice.

2012CodeofEthics

    All Cheerleaders must have a complete physical by an MD or DO within the 2012 calendar year before attending practice.
    This form can be submitted with the registration, mailed to the PO Box above or turned in at the equipment hand out or paperwork check-in on the first day of practice.
    THIS FORM IS NOT TO BE USED FOR FOOTBALL PLAYERS! FOOTBALL PLAYERS MUST USE THE TRIPLICATE FORM THAT IS AVAILABLE AT THE REGISTRATION RALLY OR AT THE BEAVERCREEK LIBRARY AFTER APRIL 22.
