Player/Cheerleader must be accompanied by a parent or legal guardian. Please bring:

ORIGINAL birth certificate and 1 photo copy

Proof of residency (ELECTRIC, WATER or GAS BILL) Original and 1 photo copy

Copy of final 2008/2009 school report card (June, July)

If you do not have the final report card (all 4 quarters), it will be collected on the first day of practice, August 3rd.
