PRE-PARTICIPATION FORMS
Players who are going to tryout for Varsity or Junior Varsity in the Spring must print out all of the forms and have them completed and turned in prior to tryouts. You can turn them in to Jen Wherley or you can bring them to the tryouts on January 11.

If you do not submit ALL of these completed forms, with ALL signatures as required, do not come to tryouts.

If you do not have personal insurance, you can purchase insurance from School Insurance of Florida for as little as $12. Go to the front office of the school for the form. To show proof of insurance, you must make a copy of the enrollment form and a copy of your check and submit those copies with the proof of insurance form below. Be sure to promptly mail the enrollment form and check to School Insurance of Florida. Insurance is effective on date postmarked.

Pre-Participation Forms

Important to all Players. You MUST print ALL of these forms and get them completed asap if you want to tryout for Varsity or Junior Varsity in January.

Make sure that ALL signatures are obtained from both parents and the player. If one parent has full custody, only their signature is required so put "NA" on the other line.

Bring these forms with you to tryouts on January 11, 2010, or you will not be allowed to tryout.

ATTENTION: The only form that needs to be notarized is the Transportation section of the Permissions Form. None of the other forms need a notary.
