Complaint/Complement Form
If you have a complaint and/or compliment concerning the operation of you local Little League, or District 51 in general, please feel free to fill out the attached form.
Your name will be kept confidential unless you allow the District Administrator to reveal your name to the League concerned.
Thank you for your interest in your local League!
Volunteer Evaluation Form
If you have a complaint and or complement concerning an individual volunteer in your local Little League or the District 51 Staff, please fill out the attached form.
Once the District Administrator receives the filled out form, you will be contacted if you so request.
Please note that this is NOT the volunteer application form required by Little League Baseball, Inc.
Thank you for taking the time to evaluate our Little League volunteers.
District 51 Registriation Form
If you live within the boundaries of District 51 and do not know which Little League you should register with, please submit the attached form to the District Administrator.
You will receive notification as to the proper Little League to register with.

Umpire Information Form
If you are a volunteer umpire for District 51, please go to the hadouts section and fill out the umpire information form and click on "SUBMIT". It should generate an email to the District Administrator so he can add you information to the Umpire database.
