ALL PARTICIPANTS THAT REGISTERED AT THE BANQUET AND OPEN SIGN-UP MUST FILL OUT THE NEW PAPERWORK AS WELL.
THE OLD MEDICAL FORM IS FINE IF YOU HAVE ALREADY HAD THAT FILLED OUT BY THE DOCTOR.

PAPERWORK CHECKLIST
Mandatory Parent Meetings>Provides the opportunity to supply the necessary paperwork and answer any questions you might have. Parents are required to attend at least one of the meetings held at the Darlington Braves Hall @ 7:00pm on the following dates...
Tuesday, June 30th - Wednesday, July 15th - Thursday, July 30th
***Each player must provide the following in order to participate***
--2009 Registration Form (must be complete with parent and participant signature)
--Original/Certified Birth Certificate or Passport (No copies allowed with out official seal) Required for all participants, veterans as well as new participants.
--2009 Medical Clearance Form(Must be signed or stamped by Doctor after January 1st, 2009)
--Report Card (Must contain grades for the entire 2008-2009 school year AND participant must have a 70 or above average)
--Image Release Form
--Liability Release Form
--Medical Consent Form

IF THE PROPER PAPERWORK IS NOT RECEIVED YOUR CHILD WILL BE REMOVED FORM THE ACTIVE ROSTER.
Doors open a 4:30 PM and bingo starts at 6:30 PM.
