The attached forms are required to particpate in Pop Warner.  In addition to these forms we will need a copy of the 2010-2011 final report card.  If this is your chilid's first year with D-R Jr Football and Cheer we will also need a certified birth certificte.  This will be returned to you at the end of Aug begining of September.
