If you are a returning player, who has turned in a registration packet last year, and none of your Insurance, or Emergency Contact info has changed. You have only to turn in a signed Code of Conduct, and Participant Waiver Form, which you can bring to Parent Meeting/Registration, or turn in to your team rep at practice.
For those NEW PLAYERS who cannot make the Registration/Parent meeting, or who would like to come prepared (highly recommended) with forms already filled out, the packet is available to download & print below. Please bring the required forms & payments to the Parent Meeting & Registration, (details to be announced soon). or give to your team rep at practice.
