2012-2013 Tryout forms
Please print out & complete two (2) copies of the "CVVC Player Tryout Application & Release Form," two (2) copies of the "2012-13 USAV Youth & Junior Volleyball Player Medical Release Form" and one (1) copy of the "CVVC Waiver & Release of Liability Form - 2013 Season." Parent or legal guardian MUST accompany player to tryouts. Please be sure that the player and the parent/legal guardian sign all pertinent places on the forms. Completing the forms ahead of time greatly helps to expedite the registration process on tryout day.
