CVI Injury Report
For all Managers and Coaches :
In the event that you have a player injured during a game or practice, you must fill out the attached form and return it to your Division Coordinator as soon as possible.
You can e-mail the completed form as well to the following :
Intermediate Division : intermediates@cvisportsleague.org
Junior A Division : junior-a@cvisportsleague.org
Junior B Division : junior-b@cvisportsleague.org
Junior C Division : junior-c@cvisportsleague.org
