Lighting Audit Form

Requires Acrobat Reader. This form is designed to be printed on both sides of an 11" by 17" sheet of paper. This is the first of two forms required to be completed and submitted annually (once before regular season play and once again prior to tournament season) for each field using artificial lighting during practices or games.

Lighting Safety Audit Form
