Regulation II(d) Waiver Form
Requires Acrobat Reader. Use this form to obtain waivers for players who have moved outside your boundaries (Tee Ball & Minor League players must have an older sibling participating as a Major League or higher level player).

Regulation IV(h) Waiver Form
Requires Acrobat Reader. Use this form to obtain a waiver for a player who has moved outside of your boundary and whose parent has lived lived within your jurisdiction and has been and will continue to be a dedicated manager, coach or board member in your league for two or more consecutive years. You can use your computer to fill in this form and then print it!
