Medical release, accident, liability and insurance claims.
Medical Release Form

Requires the Acrobat Reader. It would be a good idea to have parents fill out and sign this form at registration. The form authorizes league personnel to obtain medical treatment for an injured player whose parent is not present. The forms should be kept in the possession of team managers during regular season and tournament play. You can use your computer to fill in this form and then print it!
