1.Original birth Certificate
2. Three ( 3 ) Proofs of Residency
Residence shall be established by documents dated between February 1, 2012 and February 1, 2013 from 3 or more of the following categories.
1. Driver's License
2. Voter's registration
3. School Records
4. Welfare/Child Care Records
5. Federal, State Records
6. Support Payment Records
7. Homeowner or Tenent Records
8. Utlility Bill ( electric, water, phone) ONLY 1 allowed
9. Financial (loan) records
10. Insurance Documents
11. Medical records
12. Military Records
13. Internet, Cable or Statelite Records
14. Vehicle records
15. Employment Records
