This is a collection of the CNLL Player Registration form, Medical Release form and Parent Volunteer form. Please fill out BOTH sides of the Registration form and two copies of the Medical Release form with an original parent signature on each form. Also please include a copy of the Volunteer form. Bring all completed forms along with 3 pieces of Proof of Residency (examples include: valid Drivers License, current Auto Registration and PG and E bill - all showing the same address) AND your players ORIGINAL Birth Certificate - CNLL will make a copies of them at Signups.
