Pre-participation paperwork
Before anyone participates in any Cardinal Mooney athletic activity (pre-season conditioning, try outs, practice, games ...) they MUST have the following on file with the Athletic Director:
1. A physical
2. A permission form(4 pages)
3. A HIPPA form
Forms can be picked up in front of the AD's office in the gym.
All of these need to be completed once each school year. If you fill everything out for volleyball, you don't need to get another physical when you play basketball.

Congratulations Lacy Janson '01
Winston-Salem, N.C. - The 2006 Atlantic Coast Conference Outdoor Track and Field Championships started on a great note for redshirt senior Lacy Janson who won her fourth crown with a height of 15'0.25" (4.58m) in shattering a handful of records and moving up in the world rankings as the number two pole vaulter on the planet.
Janson had a banner day, winning her fourth Atlantic Coast Conference crown at the outdoor championships meet in grand fashion -- setting the NCAA, ACC-meet, FSU all-time, and Kentner Stadium track records. The five-time All-American becomes the second women in NCAA history to clear 15-feet and holds the highest collegiate mark ever. UCLA's Chelsea Johnson cleared 15-feet (4.57m) in 2004.
Janson is two weeks removed from her Texas Relays event win, that set the previous school record at 14'7.25" (4.45m). She is the only woman in league history to win four indoor and outdoor pole vault titles.
Congratulations Nicole Motycka '01 .....
.....and the other members of the Michigan Wolverines Softball team.
On Wednesday June 8th they captured the NCAA's Women's College World Series Title.
She might be the first female Cougar to be part of a Division I Championship Team.
