Cocalico Midget Football Association: Welcome

2013 Registration  
Registration took place on March 26th and 27th at the Cocalico Middle School. Registration is currently open. To register, download all forms on the "Handouts" page under registration. Complete and sign the Registration Form, the Code of Responsibility Form, the Fundraising Form, and Volunteer Form. Mail the four completed forms and payment to CMFA, P.O. Box 72, Denver, PA 17517. Note: CMFA has changed its jersey style moving forward and consequently all football registrants will need a new jersey. Also, note the registration fees increase on June 1st and again on July 1st. Registration closes July 31st. The first part of the Physical form must be completed by the parent/guardian and the second part by a physician. No physical may be completed prior to June 1st and a completed physical form must be returned to CMFA prior to any child participating in any practice.
