Dave Middleton
In memoriam of our dear friend and colleague, Dave Middleton, who served as Central Marin Charger’s Scholastic Director for the past two years. Those who had the pleasure of knowing Dave, remember him as the very kind and dedicated man he was. A native of Alameda, he was a resident of San Rafael, when he passed away earlier this month at the age of 51 years.
As Dave was elected President of Central Marin Chargers for the 2009 season, we are honoring his memory and service to CMC by continuing In Memoriam, with Tom McCallister as acting Vice President, and Barbi Penaflor as Acting President.
Dave is survived by his four children: Dylan, Rebecca, Nathan, and Angela; as well as his mother, brother, and sister, Dorothy, Patrick, and Sherylanne. Dylan, 12 years of age, has been playing football since 2006, and his daughter, Rebecca, now 13, was a Midget cheerleader in 2008. Those of us blessed to know Dave, remember him as the engaging and congenial guy who always brought an incredible spirit and integrity of pride to whatever projects he undertook.
If you knew Dave like we did, you will agree that we would be remiss not to also mention, Floyd, his beloved canine companion of 3 years. Floyd also survives his master and is currently living with Nathan.
CMC will be hosting a gathering a gathering in Dave’s memory (date and location information to come). All are welcome to join us in this celebration of Dave Middleton’s life. In the meantime, Dave and his family remain in our hearts and prayers.

Registration Documents (from league page)
- 2009 Player Parent Contract and Adult Code of Ethics - (click here to download)
- Both pages of this document must be completed in full.
- One parent/guardian must sign page 1.
- Signatures of both parent/guardians and the athlete are required on page 2.
- Birth Certificate - 2 copies of the athlete's birth certificate should be provided on an 8.5 x 11 inch page. Specifics:
- This document should be a copy of an official government birth certificate provided by the state or country.
- International birth certificates require an official translation if they do not provide English descriptions.
- An official court document for name change must accompany a birth certificate listing a name no longer in use.
- Hospital or baptismal announcements are not acceptable.
- One copy is kept in the certification book. The other copy should be collected after certification and kept by the association.
- 2008 to 2009 End-of-Year Report Card - 2 copies of the athlete's end-of-year report card should be provided on an 8.5 x 11 inch page. Specifics:
- Must be submitted by July 1, 2009
- Only an end-of-year report card is acceptable.
- If the report card is larger than 8.5 x 11 inches (e.g., it is legal-sized), it should be reduced to an 8.5 x 11 inch sheet of paper to fit in the certification notebook.
- A full-year report card (e.g., all 3 trimesters) must be provided for consideration for academic honors.
- The report card must have the full name of the athlete as well as the name of the school clearly visible.
- The report card must have the school year (e.g. "2008 to 2009") clearly visible.
- If the GPA is printed on the report card it must be used for scholastic monitoring.
- A GPA below 2.0 requires scholastic monitoring.
- One copy should be kept in the certification book. The 2nd copy can be used for the Academic All-American Scholars program.
- Completed Player Packet Forms - Includes the following forms:
- Image Release - Minor
- Waiver and Release of Liability-Minor
- Emergency Medical Treatment, Consent and Information
- Medical Clearance Form
Congratulations to the Central Marin Chargers Midget and Pee Wee Cheer teams for their amazing accomplishments at Nationals during the last weekend in February. Both teams are National Champions and the Pee Wees received the Grand Champion Award. Here is the video of the Midget's winning routine.
Both teams proudly dedicate their Championship Titles to Dave Middleton, President of Central Marin Chargers 2009, who unexpectedly passed away while attending the National Cheer Championship at Disneyland.
