The complete registration packet is now available in a PDF format; all the club forms to register a player are now provided (including the OYSAN and US Club player registration forms). Simply download the packet on your computer and print the forms; please complete each form and return them to either your team manager or Mrs. Kotnyski along with a small wallet photo of your son/daughter. Please make sure you have the appropriate forms notarized.
